Community Health Needs Assessment

Chester Township Health Department, in collaboration with The Morris Regional Public Health Partnership (MRPHP), and The College of New Jersey (TCNJ), is currently involved in the development of both a county-wide Community Health Assessment (CHA) and Community Health Improvements Plan (CHIP). These tools will assist our efforts in providing targeted programming/initiative for your residents and improve our ability to obtain grant funding.

You can provide an informed perspective on the quality of health in your community pertaining to healthcare, transportation, and additional factors. Your involvement will assist in the development/promotion of county and local initiatives to improve the health and lives of people within your community.
